Patient Transport Survey Toolkit

The ICB is working with NEAS to review patient transport across the North East. 

We are asking for feedback about proposed changes to the rules for who can use the patient transport service in the North East based on national guidance from NHS England, as well feedback on wider service improvements. 

We'd like to encourage feedback from users of NHS patient transport services, their carers and families in the North East area.  Please note: North Cumbria patient transport services are not included in this survey.
